Participant declares and authorises consent for
The collection and release of the above personal information and subsequent assessments about myself to Manipadma Foundation. This consent may extend to service providers, including activity leaders, specialists, assessment personnel but only to the extent required during the scope of this workshop and its implementation in my life.
Manipadma Foundation legal obligations
Manipadma Foundation will at all times safeguard the privacy of participants with regard to the collection, use and storage of the information released in accordance with this consent form.
Ethics and Confidentiality
1. Manipadma Foundation agrees not to disclose any information of workshop participants beyond the workshop’s functional need and to treat all data confidentially.
2. Manipadma will store all data from the survey in a password-protected environment.
